Social Drivers of Health Grants

Projects must last between 12 and 36 months and begin upon receipt of funding. Funding requests are limited to no more than 20 percent of an organization’s annual operating budget. Allowable expenses include program related costs and small equipment up to a limited amount, while capital projects, large equipment, political activities, travel, and debt reduction are explicitly excluded. Eligibility is restricted to nonprofit organizations with 501c3 designation or equivalent government status. Applicants must either be located in or provide services within specific counties across five states, though physical location is not required if the program demonstrates clear impact in eligible areas. Organizations may submit only one application per grant cycle and may not have more than one active grant at a time. Additional restrictions apply to organizations that have recently received funding, requiring an 18 month waiting period or completion of prior grant obligations before reapplying. The application process is conducted through the Versaic online portal. Applicants must create an account, complete an application form, and upload required documentation including audited financial statements from the past two years, organizational and program budgets, a board list, and key staff information. The audit must be complete and independently prepared, as incomplete or unaudited financials result in disqualification. The application is reviewed based on financial health indicators such as debt ratio and profit margin, as well as program design and expected outcomes. Applications are accepted during two annual cycles. The spring cycle runs from February 1 through March 1, with board decisions announced in May. The fall cycle runs from July 1 through August 1, with decisions communicated in October. Following approval, organizations must sign a grant agreement before receiving funds. Grantees are required to track outcomes and submit a report approximately one year after funding, demonstrating measurable impact. The foundation may also conduct monitoring visits or reviews at any time during the grant period.
