Application components include contact and organizational information, a signed Authorized Representative Acknowledgement Form, a current board list with residency data, a board-approved treasurer's report or audit, a detailed budget with requested funding and additional resources, and comprehensive information about the target population. Applicants must clearly identify which Community Funding priority is being addressed, describe how their service meets that need, and provide data to support the identified issue within Goodyear. Additionally, sustainability plans and service access points must be included. Program impact is assessed through detailed reporting requirements, with progress and financial updates due twice during the year—mid-January and mid-July—via the eCivis platform. Applicants must quantify the number of Goodyear residents or households served during the previous year and the projected impact of the proposed grant. Only one of the following reporting units is requested: unduplicated households, duplicated households, unduplicated residents, or duplicated residents, with clear definitions provided for each. Applications are reviewed first by city staff for eligibility and completeness, followed by a subcommittee of three City Council members who discuss and recommend funding. These recommendations are submitted to the full City Council for approval during a Regular meeting.
