Creating Health Access for Next Generation Equity (CHANGE) Program

The AstraZeneca Foundation, a philanthropic arm of AstraZeneca, launched the Creating Health Access for Next Generation Equity (CHANGE) program in 2023 to advance health equity across the United States. The Foundation focuses on addressing social drivers of health, which include the environmental and socioeconomic conditions that influence individuals’ health outcomes. Through the CHANGE program, the Foundation aims to ensure that underserved populations have improved access to timely, high-quality healthcare services by supporting community-based organizations working directly within affected communities. The primary objective of the CHANGE program is to fund initiatives that improve access to quality healthcare for populations experiencing health disparities. These disparities may be driven by factors such as limited financial resources, lack of insurance, geographic isolation, language barriers, and insufficient provider availability. The program emphasizes innovative, community-centered approaches that not only expand access to care but also address systemic barriers that prevent equitable healthcare delivery. Funded organizations are expected to operate at the community level and collaborate with other stakeholders to maximize impact. Funding through the CHANGE program is designed as general operating support, allowing organizations flexibility in how funds are used to achieve program goals. Eligible activities include providing direct healthcare services such as screenings, wellness and prevention programs, coordinating care through case management or health coaching, and mitigating barriers like transportation, affordability, and accessibility. However, funding cannot be used for capital campaigns, research or clinical trials, or formal medical education programs. The program encourages sustainability, with the expectation that funded initiatives will continue beyond the grant period through diversified funding strategies. Eligibility is limited to U.S.-based nonprofit organizations with 501(c)(3) status, as well as public schools, government entities, and municipal organizations that can accept tax-deductible contributions. Organizations must operate in at least one of the following states where AstraZeneca has a presence: California, Delaware, Maryland, Massachusetts, or Texas. Additional requirements include having total annual revenue of $12 million or less and at least three years of relevant programming experience, unless a compelling case is made for newer initiatives. Organizations must demonstrate a clear focus on improving healthcare access for underserved populations and tailoring services to community needs. The application process begins with submission of an online grant application by May 5, 2026 at 5:00 p.m. Eastern Time. Applicants are encouraged to review a preview of application questions and prepare required information in advance. Selected applicants will be notified in early June 2026 and invited to submit a full application through the CyberGrants platform between June 10 and July 8, 2026. Final award decisions are made by the Foundation’s Board of Trustees, and successful applicants will be notified in September 2026, with public announcements occurring in fall 2026. Grants range from $200,000 to $220,000 for the first year, with the possibility of renewal for up to two additional years contingent upon performance, funding availability, and board approval. Funded organizations are expected to begin grant activities by November 1, 2026 and complete the first year by October 31, 2027. Awardees must participate in reporting, evaluation, and collaborative learning activities, including monthly engagement in a Community Advisory Board. The program is structured as an annual cycle, with future funding rounds anticipated based on continued Foundation priorities.
