The Intermountain Community Care Foundation (ICCF) is a private funding organization committed to supporting Intermountain Health’s identified priorities for improving community well-being. The Foundation’s mission is to improve access to healthcare services and promote healthy behaviors for low-income, uninsured, or medically underrepresented populations in Utah and in Cassia and Minidoka counties in Southeast Idaho. It achieves this mission by funding programs and services that align with findings from Intermountain’s Community Health Needs Assessment (CHNA), which identifies significant community health needs, especially for underserved residents. Funding is offered in several focus areas. The Agency Health Priority category supports programs aimed at improving mental well-being, preventing chronic illness, and enhancing access to healthcare. The Social Drivers of Health category funds eligible agencies that align with Intermountain’s priorities in addressing non-medical factors that influence health outcomes. The Child & Family Mental Well-Being category focuses on programs that support mental health for children and families. The Education Innovation category supports K–12 education initiatives in Utah that enhance access to quality education, improve employment prospects, and foster cognitive development. This includes subcategories such as mental and behavioral health programs, early childhood education, technology and infrastructure support, community-based youth programs, teacher training and professional development, Positive Behavior Interventions and Supports (PBIS), special needs education, and higher education access initiatives. Eligible applicants must operate programs that fit into one of the identified funding categories and serve the Foundation’s target populations. Applicants must be able to provide audited financial statements dated within two calendar years of applying, proof of 501(c)(3) status, itemized organizational and program budgets, a current board of directors list, and a key staff list for personnel directly involved in the funded program. Applications must be submitted electronically through the Versaic platform, which requires account creation and registration within 24 hours of receiving the activation email. Religious organizations are eligible only if their programs are offered to the public on a nondiscriminatory basis without requiring worship. The Foundation does not fund capital projects, political campaigns, individuals, fraternal or school athletic organizations, general operations for organizations heavily funded by United Way, debt retirement, travel expenses, other foundations, media or advocacy campaigns, fundraising events, services reimbursed by third parties, SNAP doubling programs, or programs exceeding 20% of an organization’s annual operating budget. The application process includes two annual cycles. To be considered at the May board meeting, applications must be submitted between January 1 and February 1. To be considered at the November board meeting, the application window is July 1 to August 1. Funding is limited, and approval is not guaranteed even if criteria are met. Approved applicants will receive a grant agreement to be signed before payment, which is typically made in full and may be delivered by mail or in person. Grants are to be used exclusively for the purposes stated in the application, and substantive changes require prior approval. Grantees must track and report measurable outcomes approximately one year after receiving funds, and the Board or staff may review or visit funded programs at any time. Organizations may not apply again until 18 months after funding, or for multi-year grants, until the final outcomes report has been reviewed. Only one active grant per organization is allowed at any time. Evaluation of applications is conducted by a committee appointed by the Foundation President, with final decisions made by the Board. Factors considered include alignment with funding priorities, innovation, potential for lasting change, specificity, cost-effectiveness, and measurable results.
