Grant Description

Horizon Public Health, in partnership with Douglas, Grant, Pope, Stevens, and Traverse counties, is offering Opioid Settlement Funding to support local opioid response efforts. Horizon Public Health describes its role as reducing the impacts of opioid use disorders and supporting community partners in reducing stigma, reducing risks, promoting harm reduction, and strengthening protective factors. The funding is connected to Minnesota opioid settlement resources and the role of local public health departments as Chief Strategists under the Minnesota Opioids State-Subdivision Memorandum of Agreement. The purpose of the funding opportunity is to identify qualified organizations to implement opioid response strategies. Supported projects may address prevention, treatment, recovery, harm reduction, community education, family support, housing stability, access to services, care coordination, and sustainable high-impact work with measurable outcomes. Priority is given to projects that demonstrate strong community impact, especially in communities disproportionately affected by opioid-related harms. Funds may be used for costs directly related to approved project activities. Allowable expenses include staffing and benefits, training, conferences, professional development, travel tied to project activities, printing, outreach and communication materials, equipment necessary for the project, youth or community stipends, event-related costs that support broader program goals, capital improvements or construction, and other expenses that clearly support project goals. Administrative costs, including direct and indirect administrative expenses, are expected to be a small portion of the overall budget and should be 10 percent or less. No cost share or matching requirement is stated. Eligible applicants include nonprofit and for-profit organizations, healthcare organizations, government entities, schools and youth programs, faith-based organizations, neighborhood or community-based organizations, businesses, and organizations serving specific cultural, affinity, or geographic populations. A single application may request funding from one or more participating counties, but each county reviews the request independently and makes its own funding decision. The deadline is June 15, 2026, at 4:30 PM, and late or incomplete applications will not be considered. Required application content includes applicant information, county or counties where funding is requested, total amount requested, an organization overview, project narrative, work plan, detailed budget, and budget narrative. Douglas County applicants must also include the required insurance coverage form. The project narrative must describe the strategy or category addressed, key activities and timeline, implementation and impact, staffing and subcontractors if applicable, populations served, barriers and stigma, community need, and sustainability beyond the funding period. Applications will be reviewed through a competitive process. Scoring criteria include strategy alignment, impact and outcomes, community and equity, feasibility and work plan, organizational capacity, sustainability, and budget justification. County-level Opioid Taskforces conduct the initial review and may contact applicants for additional information. For Douglas, Grant, and Pope counties, applications also receive a County-level Board work session review before formal recommendation. Final funding decisions are made by the applicable County Board of Commissioners. Approved applicants will finalize implementation details, scope, timelines, reporting expectations, and a formal agreement, and must submit a W-9 and detailed invoice before funds are released. Funded organizations must submit an annual Project Summary Report describing contact updates, fund use, measurable outcomes, and project impact.
