Access to Health Care Community Grant

The Access to Health Care Community Grant, offered by the Oklahoma City Community Foundation, is dedicated to supporting organizations that enhance access to essential health care services for underserved populations. With a focus on expanding availability for individuals who would otherwise lack access to care, this program is structured to meet significant community health needs through direct service delivery. The grant prioritizes funding for initiatives in four core areas: primary, diagnostic, or preventative health services (including medical, dental, mental health, and prescriptions); early detection and screening of chronic conditions; therapeutic support for individuals with Parkinson’s disease and other serious mobility disorders; and hospice or palliative care. Funding is intended to remove barriers and enhance access to care, particularly for services offered at low or no cost to recipients in Canadian, Cleveland, or Oklahoma counties. Eligible applicants must be 501(c)(3) nonprofits actively providing services in the eligible counties. They must also show full board participation in mission-related support and meet prior grant compliance requirements with OCCF. Only one letter of intent may be submitted per organization per cycle, and past recipients may reapply provided they’ve submitted a final report for prior awards. Funding cannot be used for individuals, research, capital campaigns, vehicle purchases, or retroactive expenses. The application process begins with a Letter of Intent (LOI) due by December 4, 2025, at 5:00 p.m. Applicants whose LOIs are approved will be invited to submit a full application, due December 18, 2025, at 5:00 p.m. The application portal opens October 28, 2025. An optional information meeting will be held on November 5, 2025, to help applicants understand program expectations. Grant decisions are expected by the end of February 2026. Grants may be used for general operations, programs, or project support aligned with the funding priorities. Awards can reach up to $30,000 for a 12-month period, starting no earlier than February 28, 2026, and ending by February 28, 2027. Post-award, recipients must complete a mid-term check-in and submit a final written report via the grant portal.
