Teams to Promote Aging in Place (TPAP)

The New York State Office of Mental Health is offering the Teams to Promote Aging in Place in OMH Supportive Housing, or TPAP, Geriatric Demonstration Project under Statewide Financial System Identifier MH263034. The project is intended to help residents of OMH supportive housing remain in their communities and age in place according to their priorities and preferences. The target population includes residents age 55 and older who are at risk of losing community tenure because of unmet or emerging homecare needs, a need for more intensive cross-service system care coordination and navigation, or insufficient connections to community and natural supports, including resources addressing social determinants of health and social engagement. OMH supportive housing residents younger than age 55 may also be served when they have complex co-occurring medical conditions necessitating homecare. OMH intends to make six awards of 300000 dollars annually for five years, providing 1500000 dollars in total funding per award and 9000000 dollars across the program. At least two awards will be made in rural counties. Funding may support an enhanced homecare rate, at least one full-time-equivalent paid certified or provisionally certified mental health Peer Specialist or substance use or gambling Recovery Peer Advocate, enhanced care coordination, community partnerships, and wrap-around services and goods that support aging in place but are not covered through existing reimbursement channels. Examples of wrap-around support include non-medical transportation, community or gym memberships, continuing education, supplies supporting personal interests and hobbies, mobility aids, technology equipment, internet access, online engagement resources, and technology literacy classes. The indirect cost or administrative overhead rate is capped at 15 percent, federal Negotiated Indirect Cost Rate Agreements are not allowable, and travel must conform to New York State reimbursement rates. Eligible applicants are not-for-profit agencies with 501(c) incorporation that provide OMH supportive housing through scattered-site or single-site programs. Ineligible agencies may participate as partners, but the eligible OMH supportive housing provider must submit the proposal, receive the grant funding if selected, and assume responsibility for fiscal, contractual, and program operations. Applicants must establish an interdisciplinary care team consisting of an OMH supportive housing case manager, a local Office for Aging case manager, a homecare professional, and at least one qualified peer professional. Applicants must submit Letters of Commitment from the local aging service system provider and at least one homecare agency. Applicants must also notify the Local Governmental Unit for every county to be served and upload proof of notification. All not-for-profit applicants must be successfully prequalified in the Statewide Financial System before they can submit their bid response. Applications must be submitted through the Statewide Financial System. Mailed, hand-delivered, emailed, and other electronic submissions are not accepted. OMH advises applicants to begin working on the application no later than five business days before the deadline and to submit no later than 48 hours before the deadline when possible. Applicants must use the proposal template supplied in the Event Comments and Attachments section of SFS. The proposal template and supporting attachments must be combined into one continuous PDF and uploaded under Question 1, while the five-year Operating Budget, Appendix B, must remain in Excel format and be uploaded under Question 2. Required materials include proof of Local Governmental Unit notification, the Provider Contact form, Sexual Harassment Prevention Certification, Gender-Based Violence and the Workplace Certification, the proposal narrative and applicable attachments, Letters of Commitment, and the Operating Budget. A workplan is not required at the application stage. The proposal narrative addresses Notification of LGUs, Statement of Need and Population to be Served, Description of Program, Description of Community Partnerships, Description of Training and Education, Implementation, Agency Experience, Diversity Equity and Inclusion and Resident Input, and Financial Assessment. Proposals are scored on a 100-point scale. Notification of LGUs is worth 1 point, Statement of Need and Population Served 15 points, Description of Program 15 points, Community Partnerships 10 points, Training and Education 10 points, Implementation 10 points, Agency Performance 9 points, Diversity Equity and Inclusion and Resident Input 10 points, and Financial Assessment 20 points. Proposals scoring below 70 are eliminated. The two highest-scoring rural proposals will be selected first, followed by the four highest-scoring remaining applications regardless of rural or non-rural designation. A scoring tie is resolved in favor of the proposal receiving the higher Description of Program score. The RFP was released August 27, 2026. A Bidder's Conference is scheduled for September 10, 2026, from 10:00 AM to 11:00 AM. Questions are due September 17, 2026, and official answers are scheduled for posting October 8, 2026. Proposals are due October 29, 2026, by 2:00 PM EST. The anticipated award notification date is December 10, 2026, and the anticipated contract start date is July 1, 2027.
