Pathways to a Healthy Kansas Grant Program

Eligibility is limited to established community health coalitions operating within Kansas that are registered with Elevate Kansas. These coalitions must demonstrate organizational capacity, stakeholder engagement, and a commitment to advancing health equity. Applicants must coordinate efforts across sectors, identify populations of focus experiencing health disparities, and engage those populations in planning and implementation. The application process begins with a required Letter of Intent, followed by an invitation-only full application for selected coalitions. The submission process includes completing a Letter of Intent form by August 21, 2026, after which eligible applicants receive access to the full application. The final application deadline is October 2, 2026. Selected applicants may undergo site visits between November 2 and November 20, with final award notifications issued on December 1, 2026. The grant period begins January 1, 2027, and runs through December 31, 2028, with a kickoff event scheduled for spring 2027. Reporting requirements include mid-cycle and final reports, as well as ongoing evaluation and data collection activities coordinated with program partners. Evaluation criteria emphasize coalition readiness, cross-sector collaboration, commitment to equity, and the ability to implement and sustain community-wide strategies. Technical assistance is a core component of the initiative and is provided by organizations such as the Kansas Health Institute and the Community Engagement Institute.
