Organizations must demonstrate alignment with the foundation’s mission by offering services such as healthcare access, case management, health navigation, mental health counseling, transportation to medical appointments, or preventative health screenings. Applicants must also meet a fiscal requirement, with annual operating budgets not exceeding $300,000. The funding ceiling for this opportunity is $5,000 per grant. While the fund is flexible in supporting general operations or programmatic needs, it does not support activities such as aid for individuals, medical research, Medicare-focused services, or single-disease programs. Endowments, fundraising activities, and deficit funding are also explicitly excluded. The application period for the most recent cycle has closed.
