Cooperative Agreement to Support Navigators in Federally-facilitated Exchanges
$90,000,000
HHS-CMS (Centers for Medicare ; Medicaid Services)
Federal

Application Deadline

Jul 1, 2024

Date Added

Jul 31, 2023

The Cooperative Agreement to Support Navigators in Federally-facilitated Exchanges (Funding Opportunity) will enable recipients to operate as Exchange Navigators in states with a Federally facilitated Exchange (FFE), as authorized by the Affordable Care Act (ACA). Each year, the Centers for Medicare Medicaid Services (CMS) makes grant awards to organizations who serve as Navigators in FFE states. Navigators help consumers prepare applications to establish eligibility and enroll in coverage through the Exchanges, and help determine whether consumers potentially qualify for insurance affordability programs. They also provide outreach and education to raise awareness about the Exchanges. As trusted community partners, Navigators are required to provide targeted assistance to underserved and vulnerable populations within the Exchange service area, which is critical to improving access to health care coverage for communities that often experience a disproportionate burden of disease. In furtherance of the Biden Administrations goals on ensuring access to and expanding affordable and high-quality health care coverage and improving health equity, Navigators are well positioned to reach historically underserved and vulnerable communities, especially those who continue to face significant health disparities. Navigators operate year-roundincreasing awareness among the remaining uninsured about the coverage options available to them, helping consumers find affordable coverage that meets their needs, and assisting consumers beyond the enrollment process to ensure theyre equipped with the tools and resources needed to utilize and maintain their health coverage all year. The increased funding available to applicants through this Funding Opportunity will enable FFE Navigators to further expand their reach, with a renewed focus on outreach to diverse, underserved, and vulnerable communities.

Urban & Community Forestry Assistance Grants
$40,000
Tennessee Department of Agriculture (TDA)
State

Application Deadline

Sep 27, 2024

Date Added

Sep 20, 2024

The Tennessee Urban and Community Forestry (U&CF) Assistance Program, announced by the Tennessee Department of Agriculture Division of Forestry (TDF), is a grant opportunity provided in partnership with the Forest Service. This program aims to promote the creation, enhancement, and protection of urban and community forest ecosystems throughout Tennessee, supporting full-circle management of these valuable resources. The broader mission alignment for this program is rooted in fostering sustainable and healthy communities through environmental stewardship, which aligns with the strategic priorities of both the Forest Service and the Tennessee Department of Agriculture. The program targets units of local governments (city, town, county), non-profit 501(c)3 organizations, and public educational institutions. Community tree volunteer groups, neighborhood associations, or civic groups that are not 501(c)3 organizations are also eligible if they apply in partnership with a non-profit or local government. The impact goals are to integrate U&CF principles into planning, promote the role of U&CF in human health and wellness, cultivate diversity and leadership in the U&CF community, strengthen forest health and biodiversity, improve management and stewardship, and increase funding for U&CF initiatives. Key priorities for projects include extreme heat mitigation, urban wood utilization, urban food forests, and workforce development. The program also emphasizes increasing public awareness and environmental education to promote stewardship, as well as engaging with communities under the Justice40 initiative through outreach, education, and technical and financial assistance. This focus reflects a strategic theory of change that links targeted interventions in urban forestry to broader improvements in public health, environmental resilience, and social equity within Tennessee communities. Expected outcomes and measurable results include facilitating strong partnerships among various governmental and non-profit entities, increasing awareness and practice of full-circle management in urban forests, and improving community preparedness for natural disasters. The program also seeks to increase the number of communities participating in Arbor Day Foundation recognition programs and to increase awareness of the numerous benefits of trees, such as those related to extreme heat mitigation and stormwater management. Applicants can apply for amounts between $5,000 and $40,000, with projects expected to be completed within one year. Eligible projects encompass urban and community forestry management, tree planting, program development or capacity building, urban and community wood utilization, and workforce development.

Assessing Use of Health IT by U.S. Physicians Providing Outpatient Care
$425,000
Department of Health and Human Services - Office of the National Coordinator
Federal

Application Deadline

Jul 22, 2024

Date Added

Jun 17, 2024

The Office of the National Coordinator for Health Information Technology (ONC) is accepting applications for a cooperative agreement to measure the use of health information technology (IT) among office-based physicians. The purpose of this cooperative agreement is to measure U.S. office-based physicians' use of interoperable health IT, their experience using health IT, and the impact of health IT on the care they deliver. The cooperative agreement is intended to produce detailed data on interoperability and use of health IT by office-based physicians. These data will result in insights on the implementation and effects of federal health IT policies, allow for identification of disparities in the physicians adopting and using interoperable health IT in terms of the populations they serve or unintended consequences such as increased burden on clinicians resulting from the implementation of health IT, and will inform ONC's ongoing efforts to achieve better health enabled by data through coordination and regulation. The cooperative agreement recipient is encouraged to make the key findings of the cooperative agreement publicly available via their website; peer-reviewed publications in biomedical journals; presentations at conferences; webinars, or blog posts.
