The NHLBI expects that applications to this NOFO will describe the planned clinical trial and in so doing demonstrate that the proposed (R34) research is scientifically necessary to design or plan the subsequent trial. Furthermore, this NOFO will support research projects that are designed to provide results that will be sufficient to inform the future trial without further studies. The planned Phase II, III, or IV trial must be primarily intended to test the efficacy, safety, clinical management, or implementation of intervention(s) in the prevention and/or treatment of heart, lung, blood, and sleep disorders. In contrast to the study start up or preparation phase of NHLBI funding opportunities for clinical trials (as described at https://www.nhlbi.nih.gov/grants-and-training/funding-opportunities-and-contacts/clinical-trials-optimization), the R34 mechanism is intended to provide new information that answers a scientific or operational question(s) which may be pragmatic in nature and, therefore, informs the final development of a Phase II-IV clinical trial. Regardless of the results of the R34, support of the proposed future clinical trial will require a new application.

Recipients will focus on conducting data dissemination and educational activities to increase and expand awareness about that proposed chronic condition to support effective, evidenced-based clinical and population-level approaches. 2020, Congress recognized CDC’s work with stakeholders to expand public health education and awareness activities that help improve surveillance, diagnosis, and proper treatment for chronic diseases. Congress charged CDC with establishing a new competitive grant program to expand and advance work with stakeholders on education, outreach, and public awareness activities for a variety of chronic diseases for which there is a clear disparity in public and professional awareness and that are not currently funded through a direct FY21 CDC appropriations line. This approach would strengthen the science base for prevention, education, and public health awareness for a variety of chronic diseases that do not currently have dedicated resources that would lead to meaningful patient outcomes. This 3 year competitive grant will complement or fill gaps in CDC’s National Center for Chronic Disease Prevention and Health Promotion’s (NCCDPHP) current priorities by supporting recipients for the following activities: 1) Data dissemination activities that support awareness for public health professionals; 2) Develop data collection tools to fill surveillance gaps; 3) Educational activities supporting public awareness; and 4) Educational activities supporting health professional screening, referral, and/or management. Collecting and disseminating reliable information on the impacts of chronic diseases (including social, behavioral, environmental, and quality of life aspects) is necessary to shape effective evidence-based clinical and population-level approaches for public health impact and health equity.

The purpose of the NIH Midcareer Investigator Award in Patient-Oriented Research (K24) is to provide support to mid-career health-professional doctorates for protected time to devote to patient-oriented research (POR) and to act as research mentors primarily for clinical residents, clinical fellows and/or junior clinical faculty. Level of Effort Candidates must be able to demonstrate the need for protected time, 3-6 person months (25-50% of full-time professional effort) for a period of intensive research focus as a means of augmenting their capabilities in POR and ability to act as a mentor to new clinical investigators in the conduct of POR during this period. Candidates for the K24 award may not concurrently apply for any other PHS career award or have another PHS career award pending review. Candidates who have VA appointments may not consider part of the VA effort toward satisfying the full time requirement at the applicant institution. Candidates with VA appointments should contact the staff person in the relevant Institute or Center prior to preparing an application to discuss their eligibility. After the receipt of the award, adjustments to the required level of effort may be made in certain circumstances. See NOT-OD-09-036 for more details. Renewals Depending on the policies of the sponsoring NIH Institute/Center, awardees may apply for a one-time renewal for an additional three to five years of support if the recipient continues to have independent peer-reviewed research support at the time of submission of the renewal application. Candidates should clearly demonstrate their continuing need for protected time to expand their research programs. Peer-Reviewed Research Support Depending on the policies of the sponsoring NIH Institute/Center, candidates are expected to continue to hold independent peer-reviewed research support for the period of this award. Candidates losing this support during the award period must document in their annual Progress Reports efforts to replace this support and demonstrate that they continue to meet all other requirements of the career award. Institutional Environment The applicant institution must have a strong, well-established record of research and career development activities and faculty qualified to serve as mentors in biomedical, behavioral, or clinical research.
