The model is expected to be evidence-based, incorporating best practices for clinical interventions and participant support. The CR-SRO will provide residence-based programming that meets ongoing needs, while also coordinating with higher levels of care when necessary. The FACT team will offer individualized case support, consultative services, and crisis de-escalation assistance for residential staff. The total funding available for this opportunity is $30,021,763, and a single non-profit organization will be awarded the contract. The anticipated initial contract length is 60 months. Applications are due by May 1, 2025, at 2:00 PM (ET). There is no bidder’s conference or application workshop for this grant. Questions regarding the application process must be submitted by March 20, 2025, and responses will be posted online by April 10, 2025. Eligible applicants must be non-profit organizations with a demonstrated history of providing mental health services to individuals with SMI and justice involvement.
