NWD Systems simplify access to LTSS and are a key component of LTSS systems reform. The ADRC core services include: Specialized Information, Referral, and Assistance (IR&A); Referrals to respite care services; Local Contact Agency services to provide assistance to non-Medicaid, Medicare, and private pay nursing facility residents to transition into community living; Housing Navigator Services to increase accessible and affordable housing; and Outreach Activities under the Medicare Improvement for Patients and Providers Act (MIPPA) to educate Medicare beneficiaries with limited incomes who may be eligible for the Low-Income Subsidy program (LIS), Medicare Savings Program (MSP), and Medicare Prescription Drug Coverage (Part D), as well as disease prevention and wellness promotion.

However, the use of funds must supplement—not supplant—existing initiatives and comply with federal Uniform Guidance at 2 CFR 200. Indirect costs are allowed but are restricted based on entity type and federal guidance. Eligible applicants include Local Education Agencies (LEAs), institutions of higher education, Regional Offices of Education (ROEs), other public agencies, and private nonprofit organizations. Only one application per entity, identified by Federal Employer Identification Number (FEIN), will be accepted. Entities must complete the State of Illinois Grant Accountability and Transparency Act (GATA) prequalification process, which includes registration in the System for Award Management (SAM), and must obtain a Region County District Type (RCDT) code before submission. Applications must be submitted online as a complete PDF package by 4:00 PM on August 18, 2025. The grant opportunity was posted on July 2, 2025, and applications were accepted from that date forward. Scoring is based on need, capacity, program quality, implementation strategy, program awareness efforts, and evaluation design. Applicants must achieve at least 80 points to be considered for funding. The awarded entity will also be responsible for periodic reporting to ISBE, including monthly meetings, quarterly financial reports, and annual programmatic evaluations.

Organizations can submit for different types of enhancements within their application (e.g., locks and alarm system) so long as the total requested by the organization is equal to, or less than, $200,000. Organizations may submit applications for projects at facilities that have received any prior SCAHC funding if the proposed project is different than any previously funded projects or builds upon or enhances the security measures previously undertaken. All funding must support project related costs that will be incurred during the contract period. Funding under this program must supplement, not supplant, non-grant funds that would otherwise be available.
