The CR-SRO component of the program will consist of a 25-unit residential program offering structured support to residents with SMI and justice involvement. It will be integrated with a FACT team that has a capacity for 68 participants, ensuring both individualized case management and crisis intervention services. The model is expected to be evidence-based, incorporating best practices for clinical interventions and participant support. The CR-SRO will provide residence-based programming that meets ongoing needs, while also coordinating with higher levels of care when necessary. The FACT team will offer individualized case support, consultative services, and crisis de-escalation assistance for residential staff. The total funding available for this opportunity is $30,021,763, and a single non-profit organization will be awarded the contract. The anticipated initial contract length is 60 months. Applications are due by May 1, 2025, at 2:00 PM (ET). There is no bidder’s conference or application workshop for this grant.
